今からおよそ1300年前の白鳳時代、天武天皇が皇后・鵜野讃良皇女（うののさららのひめみこ／のちの持統天皇）の病気平癒を祈って発願され、持統天皇が即位してから藤原京に造営されました。その後平城遷都に伴い、養老2年（718）に現在地に移されました。中央に本尊・薬師三尊像をまつる金堂、東西に2基の塔を配する日本初の伽藍配置は「薬師寺式伽藍配置」と呼ばれています。その大きな特徴は、堂塔の各層に裳階（もこし）をつけた壮麗な姿の「龍宮造り」でした。
しかしその華麗な堂塔は数次の火災にあって次々と焼失し、創建当時から現存する建造物は東塔（国宝）のみとなりました。その後、長年のお写経勧進により、昭和51年（1976）に金堂が、昭和56年（1981）には西塔が、その後中門、回廊、玄奘三蔵院伽藍などが復原造営され、平成15年（2003）には大講堂が落慶、平成29年（2017）には食堂（じきどう）も再建、創建当時の壮麗な白鳳伽藍が鮮やかに復興されました。平成10年（1998）、法相宗大本山 薬師寺は「古都奈良の文化財」の構成資産の1つとして、ユネスコの世界遺産に登録されました。
近年では、国宝・東塔の12年に及ぶ全面解体修理が行われ、令和3年（2021）2月に竣工。「凍れる音楽」という愛称で親しまれる美しい姿に蘇りました。薬師寺は、白鳳伽藍とともに国宝・薬師三尊像をはじめとする多くの文化財を継承し、1300年続く祈りを今も伝えています。

During the Hakuho period, about 1,300 years ago, Emperor Tenmu made a vow to pray for the recovery of his empress, Princess Uno no Sarara (later known as Empress Jito), from illness, and it was built in Fujiwara-kyo after Empress Jito ascended to the throne. It was later moved to its current location in 718 when the capital was moved to Heijō-kyō. The layout of the temple, the first of its kind in Japan, features a main hall housing the Yakushi Triad in the center, and two towers on the east and west, known as the Yakushi-ji style temple layout. Its main feature was the magnificent “Ryūgū-zukuri” style, with a mokoshi (a raised staircase) on each floor of the temple.
However, these magnificent temples and towers were destroyed one after the other in several fires, and the only structure remaining from the time of construction is the East Tower (a national treasure). After that, thanks to many years of soliciting for the copying of sutras, the main hall was built in 1976, the west pagoda in 1981, and the middle gate, corridors, and Xuanzang Tripitaka temple buildings were restored. The great lecture hall was completed in 2003, and the dining hall was rebuilt in 2017, restoring the magnificent Hakuho temple buildings to their original state. In 1998, Yakushiji Temple, the head temple of the Hosso sect, was registered as a UNESCO World Heritage Site as one of the components of the “Historic Assets of Ancient Nara.”
In recent years, the national treasure East Pagoda underwent a complete dismantling and repair over a period of 12 years, and was completed in February 2021. It has been restored to its beautiful form, affectionately nicknamed “Frozen Music.” Yakushiji Temple, along with the Hakuho Garan, has inherited many cultural assets, including the national treasure Yakushi Triad, and continues to convey prayers that have continued for 1,300 years.
